Are you currently treated with long-term-oxygen therapy with more than 4.0 L/minute?

Have you been diagnosed with ulcerative colitis?

Have you been diagnosed with Crohn’s disease?

In the past 5 years, have you been diagnosed with cancer (not including common types of skin cancer)?
